For a groundwater system, the practical question is narrower: what activities can affect the aquifer or well before treatment has a chance to respond? A useful plan answers that question with current local evidence, not assumptions. It gives the operator, board, inspector, and emergency responder the same picture of where the source is vulnerable and who owns the next action.

The five records to keep together

  • Well and source profile. Keep the PWSID, well location, source type, construction details available to the system, and the latest water quality results together.
  • Protection area map. Use the state approved source water assessment or local hydrogeologic work to identify the area that can contribute water to the well. Do not invent a fixed radius when the state has already defined a protection area.
  • Risk inventory. List current land uses and potential contaminant sources inside or near the protection area, such as fuel storage, septic systems, agricultural chemical handling, dry wells, waste sites, or industrial activity. Update it when the area changes.
  • Controls and contacts. Record which controls apply, who is responsible, and the contact path for the utility, local planning office, fire department, health department, and state drinking water program.
  • Inspection and response record. Retain survey findings, corrective actions, spill or incident notes, sampling results, and plan revisions so the system can show what it did and when.

Do not confuse a source water plan with a monitoring waiver

A source water or wellhead protection plan helps a system prevent contamination and prepare for an incident. It does not replace the monitoring schedule issued by the primacy agency, and it does not erase a missed sample, an MCL exceedance, or a sanitary survey deficiency. Keep the two workstreams connected: the plan explains the source risks; the compliance calendar shows the samples, reports, corrective actions, and notices that are due.

Where the plan connects to a sanitary survey

Sanitary surveys examine the condition of the source, facilities, operation, maintenance, and records. A current wellhead plan makes the source water portion easier to document because the map, risk inventory, contacts, and corrective-action history are already organized. If a survey identifies a significant deficiency, keep the notice and the state approved correction timetable with the plan. Do not describe an item as resolved until the agency or the applicable record confirms it.
